ABSTRACT = { <b>Background:</b> Bladder leiomyomas are rare benign tumours arising from smooth muscle cells, accounting for less than 0.5% of all bladder neoplasms. Surgical excision remains the gold standard for management; however, few reports describe the use of robotic assistance for this purpose. <b>Case Description:</b> We present the case of a woman in her mid-30s who developed painless macroscopic haematuria and acute urinary retention, with imaging revealing a large bladder leiomyoma. Following unsuccessful endoscopic management and hysterectomy, she underwent robotic-assisted transvesical excision of the lesion, resulting in marked symptomatic improvement. <b>Conclusions:</b> Larger bladder leiomyomas can present significant diagnostic and therapeutic challenges, particularly when located near the trigone. This case demonstrates that robotic transvesical excision is a feasible and effective approach in selected patients and highlights the importance of considering primary bladder pathology in cases of diagnostic uncertainty.},
