Abstract The majority of patients with congenitally corrected transposition of the great arteries, also known as transposition of the great arteries {S,L,L} have ventricular septal defects (VSD), most commonly perimembranous VSD (pmVSD). Transcatheter device closure of pmVSD in these patients has not been widely described. We present a case of device closure of pmVSD in L-TGA with an Amplatzer Duct Occluder II (ADOII) device using a deployment starting in the subpulmonary left ventricle. The case demonstrates some of the technical advantages of the ADOII device for VSD closure, specifically its low profile, symmetric shape, and soft More >

Abstract Objective: This study aims to evaluate the morphology and function of the aortic valve after transcatheter closure of ventricular septal defect (VSD) with aortic valve prolapse (AVP) abased on clinical and radiological outcomes. Methods: From January 2013 to November 2014, 164 consecutive patients (97 males, 59.1%) with VSD and AVP were treated by transcatheter closure. The patients were divided into the mild AVP group (n = 63), moderate AVP group (n = 89) and severe AVP group (n = 12). The clinical and radiological outcomes of these patients were analyzed retrospectively. Results: In total, 146 (89.0%) patients… More >

Abstract Background: Never had literatures characterized the relationship between the property of major aortopulmonary collateral arteries (MAPCAs) and outcomes of selective unifocalization of pulmonary atresia with ventricular septal defects and MAPCAs. Methods: This is a case-series study. Thirteen patients were included. Angiography-based assessment was conducted to determine whether collateral arteries should be unifocalized or treated with intraoperative ligature. Specimens were collected and stained by HE and ET+VG. Results: Twelve patients underwent one-stage unifocalization at a median age of 37 months (range: 6–228 months) and a median weight of 14.0 kg (range: 5.0–49.0 kg), which produced a favorable right… More >

Transcatheter Closure of a Right Pulmonary Artery to Left Atrium Fistula Using a Ventricular Septal Defect Occluder

    Diqi Zhu1, Xiaohong Gu2, Jie Shen1,*, Fen Li1,*

    Congenital Heart Disease, Vol.16, No.2, pp. 147-150, 2021, DOI:10.32604/CHD.2021.014360 - 26 January 2021

Abstract Background: Communication between the right pulmonary artery (RPA) and left atrium (LA) is a rare cause of central cyanosis in pediatric patients. Case presentation: We describe a 3-year-old female patient with an oxygen saturation of 70% at admission. The echocardiogram indicated an abnormal color flow Doppler in the LA and she underwent standard cardiac catheterization. The angiography of pulmonary artery revealed a 7.4 mm × 7.6 mm fistula between the RPA and LA and achieved successful closure using ventricular septal defect occlusion. Conclusion: The fistula between pulmonary artery and left atrium is an extremely rare but treatable More >

Abstract Background: This study aims to explore the efficacy of selective unifocalization (UF) for major aortopulmonary collateral arteries (MAPCAs) unifocalization in children with pulmonary atresia with ventricular septal defect (PA/VSD). Methods: A retrospective analysis of 13 patients with PA/VSD/MAPCAs who underwent surgery from June 2017 to December 2019. Sex, age, preoperative cardiovascular angiography test results and McGoon ratio were collected. The properties of the collateral arteries were evaluated by angiography, and selective UF for the “dendritic” MAPCAs and ligation of MAPCAs demonstrating distortion and resistance. Results: A total of 13 severe patients underwent one-stage repair, of which 1 More >

Characterization of Malalignment between Atrial and Ventricular Septa in Overriding/Straddling Tricuspid and Mitral Valves

    Mika Saito1,2, Dragulescu Andreea1,2, Shi-Joon Yoo1,2,3,*

    Congenital Heart Disease, Vol.16, No.1, pp. 1-16, 2021, DOI:10.32604/CHD.2021.011694 - 23 December 2020

Abstract Objective: Overriding/straddling of an atrioventricular valve is a consequence of atrial-ventricular septal malalignment. We sought to characterize the malalignment by analogizing it to a flap-door and using structured approach. Methods: The echocardiograms and magnetic resonance images of 35 patients with overriding/straddling tricuspid or mitral valve were evaluated to assess the following modifiers; the malaligned part of the septum, the reference structure, and the mechanism, direction and severity of malalignment. Results: The pathology included classic overriding tricuspid valve in 15, classic overriding mitral valve in 12, and overriding tricuspid valve with left juxtaposition of atrial appendages in 8.… More >

Anoosh Esmaeili, Kachina Behnke‐Hall, Roland Schrewe, Dietmar Schranz

    Congenital Heart Disease, Vol.14, No.3, pp. 389-395, 2019, DOI:10.1111/chd.12731

Abstract Aim: The purpose of this study is to describe the special aspects of perimembranous ventricular septal defects (pmVSD) closure by utilizing Amplatzer Duct Occluder II (ADO II) devices with a rational request for bigger ADO‐II sizes, based on our experi‐ ence in transcatheter device closure of pmVSD.
Methods and Results: At our institution, placement of an ADO II device was used in 15 patients with pmVSD; the patients’ age ranged between 6 months and 20 years. The indications for closure were CHF (n = 4), hemodynamically significant shunt (n = 7), tricuspid regurgitation (n = 3), and… More >

    Congenital Heart Disease, Vol.14, No.2, pp. 280-287, 2019, DOI:10.1111/chd.12724

Abstract Objective: Outcomes after surgical repair of complete atrioventricular septal defect (cAVSD) have improved. With advancing age, the risk of development of dysrhythmias may increase. The aims of this study were to (1) examine development of sinus node dysfunction (SND), atrial and ventricular tachyarrhythmias, and (2) study progression of atrioventricular conduction abnormalities in young adult patients with repaired cAVSD.
Study design: In this retrospective multicenter study, 74 patients (68% female) with a cAVSD repaired in childhood were included. Patients’ medical files were evaluated for occurrence of SND, atrioventricular conduction block (AVB), atrial and ventricular tachyarrhythmias.
Results: Median age at… More >

    Congenital Heart Disease, Vol.13, No.6, pp. 1038-1044, 2018, DOI:10.1111/chd.12678

Abstract Objective: We aimed to investigate the effect of ventricular septal defect (VSD) and heart failure on cerebral blood flow (CBF) in children, whether heart failure treatment improves CBF, and if there is any relationship between CBF and serum N‐ terminal pro‐brain natriuretic peptide (NT‐proBNP) level.
Method: Forty children with VSD (13 with heart failure) aged between 1 and 36 months were studied. The control group comprised 25 healthy children in the same age group. Maximum, minimum, and mean blood flow velocities and pulsatility indices of the right and left middle cerebral arteries were assessed using transcranial Doppler… More >
