Summary

Female urethral pathologies are widespread, yet they remain underrecognized in clinical practice. Women have been treated by both urologists and gynecologists; however, there are no standardized guidelines for diagnosis, treatment, or outcomes. Furthermore, the terminology used in female urethroplasty remains inconsistent and often controversial in the literature.

In contrast to male urethral surgery, treatment options for women are less widespread, and women are often managed with incorrect procedures, all of which can cause additional trauma and further fibrosis of the urethra, potentially worsening the condition, and therefore may not be helpful.
This Special Issue aims to consolidate existing knowledge and provide a platform to highlight the need for unified, minimally invasive surgical approaches to restore urinary function while preserving continence mechanisms, sexual function, and the aesthetic appearance of the genitalia, in line with the standards established for female patients.

By addressing these gaps, we aim to deepen the understanding of key aspects of female urethral surgery and improve patient management and clinical outcomes globally.

Abstract Introductions: Female lower urinary tract symptoms (LUTS) are heterogeneous and often reflect bladder outlet obstruction (BOO) from anatomic urethral disease. Evidence guiding surgery remains fragmented. This study aimed to evaluate surgical and patient-reported outcomes after reconstructive surgery for female anatomical urethral disease. Materials and Methods: We retrospectively analyzed a prospectively maintained, single-surgeon database of women undergoing urethral surgery (January 2017–April 2024) for female urethral stricture (FUS), female urethral diverticulum (FUD), or periurethral masses. Standardized preoperative assessment was applied; procedures included dorsal inlay buccal mucosa graft (DMG), ventral onlay grafts (VOG, buccal or vaginal), partial diverticulectomy, and… More >
