Impact of a health education intervention in overactive bladder patients

    Sender Herschorn1, Debbie Becker2, Elizabeth Miller3, Melissa Thompson2, Lindy Forte3
    Canadian Journal of Urology, Vol.11, No.6, pp. 2430-2437, 2004
Abstract Objective: To assess a standardized and simple educational intervention in overactive bladder (OAB) patients to improve compliance with anticholinergic medication, increase the use of concomitant behavioral treatments, and improve patients' perception of bladder symptoms.
Materials and methods: This is a 16-week open-label randomized trial of tolterodine combined with an education intervention for the experimental group versus tolterodine alone (no intervention) for the control group. The setting was in family medicine and urology clinics in Ontario. The participants were male and female adults with OAB symptoms. Both groups received tolterodine prescriptions. The intervention patients received printed information and… More >

A prospective evaluation of the diagnostic and potential prognostic utility of urinary human telomerase reverse transcriptase mRNA in patients with bladder cancer

    Les Bowles1,2, Hanna Bialkowska-Hobrzanska1,2,3,4, Bernard Bukala5, Linda Nott5, Hassan Razvi2,5
    Canadian Journal of Urology, Vol.11, No.6, pp. 2438-2444, 2004
Abstract Human telomerase reverse transcriptase (hTERT) mRNA expression has been considered a surrogate marker for telomerase activity based on its parallel detection in urological malignancies, including transitional cell carcinoma (TCC) of the bladder. The objective of this study was to prospectively evaluate the diagnostic performance of urine hTERT mRNA marker and urine cytology in the detection of bladder cancer.
The multiplex hTERT/GAPDH (glyceraldehyde-3- phosphate dehydrogenase) reverse transcription polymerase chain reaction (RT-PCR) assay was employed to assess hTERT mRNA expression in urine sediments from 43 patients with clinically apparent TCC undergoing transurethral resection. Tumor grade and pathological stage… More >

A3-week gemcitabine-cisplatin regimen for metastatic urothelial cancer

    Eric Winquist1, John J. Wilson2, Mark Dorreen3, Ralph Wong4, Derek Jonker5, Neil Iscoe6
    Canadian Journal of Urology, Vol.11, No.6, pp. 2445-2449, 2004
Abstract Objective: To assess the efficacy and tolerability of a 3-week outpatient schedule of intravenous gemcitabine and cisplatin in patients with locally advanced unresectable or metastatic transitional cell carcinoma of the urothelial tract (TCC).
Patients and methods: A two-stage phase II trial enrolled TCC patients with Karnofsky performance status >60, measurable disease, and adequate organ function. Prior adjunctive chemotherapy was allowed provided it had been completed at least 1 year prior to study entry. Treatment consisted of gemcitabine 1250 mg/m2 iv days 1 and 8 plus cisplatin 70 mg/m2 day 1 iv repeated every 21 days. The primary outcome… More >

Success of sildenafil for erectile dysfunction in men treated with brachytherapy or external beam radiation for prostate cancer

    Oren M. Shemtov1, Sidney B. Radomski2, Juanita Crook3
    Canadian Journal of Urology, Vol.11, No.6, pp. 2450-2455, 2004
Abstract Purpose: We undertook to determine if any significant differences in the efficacy of sildenafil citrate for erectile dysfunction (ED) exists between patients who have received external beam radiation or brachytherapy for prostate cancer.
Materials and methods: Thirty-one patients who had received external beam radiation and nineteen patients who had received brachytherapy for prostate cancer and were subsequently treated with sildenafil citrate for post-irradiation ED comprised the patient population. A chart analysis was performed to determine either the presence or absence of concomitant risk factors for ED (coronary artery disease, diabetes, hypertension and smoking history) as well as… More >

    CASE REPORT

Bladder perforation during laparoscopic donor nephrectomy

    P. D. Metcalfe, L. Hickey, J. G. Lawen
    Canadian Journal of Urology, Vol.11, No.6, pp. 2456-2458, 2004
Abstract We present two cases of bladder perforation during laparoscopic donor nephrectomy at our institution. Neither of the surgeries was otherwise complicated, and the diagnoses were made post-operatively. The kidneys were extracted through a Pfannenstiel incision and used blunt dissection to penetrate the peritoneum. Both patients had previous tubal ligations, adhesions from which may have increased the chance of injury. We believe that this is a previously unreported complication that merits attention. Care should be taken with the peritoneal incision and dissection as the bladder may be susceptible to injury. More >

Post-pyeloplasty flank pain treated with laparoscopically-assisted renal autotransplantation

Iliana C. Lega, Gerald B. Brock, Patrick P. W. Luke
    Canadian Journal of Urology, Vol.11, No.6, pp. 2459-2460, 2004
Abstract Despite excellent overall results, some patients continue to experience flank pain post-pyeloplasty. For the first time, we report the successful use of laparoscopically-assisted renal autotransplantation in the treatment of refractory flank pain post-pyeloplasty. More >

Synchronous transitional cell carcinoma and metastatic hepatocellular carcinoma in the urinary bladder: a case report

    Nabeel Al-Brahim1, Salem Alowami1, Ian Davis2, Dean Daya1
    Canadian Journal of Urology, Vol.11, No.6, pp. 2463-2466, 2004
Abstract An 83- year-old male presented with intermittent hematuria and obstructive symptoms. A CT scan of the abdomen and pelvis showed diffuse thickening of the urinary bladder and a large 10 cm liver mass. Histopathological examination of bladder biopsy demonstrated two distinct lesions. The surface showed non-invasive urothelial carcinoma and the submucosa demonstrated metastatic hepatocellular carcinoma showing bile pigment and expressed Hep Par 1 and CD10. To the best of our knowledge, this is the first case report of synchronous transitional cell carcinoma and metastatic hepatocellular carcinoma of the urinary bladder. More >

Isolated late recurrence of renal cell carcinoma in the inferior vena cava

    David C. Horger1, Nabil K. Bissada2, Nancy S. Curry3, Uzair B. Chaudhary4
    Canadian Journal of Urology, Vol.11, No.6, pp. 2467-2469, 2004
Abstract The occurrence of an isolated late recurrence of renal cell carcinoma within the inferior vena cava (IVC) is a rare event. We present a case of a 55-year-old patient with recurrence in the IVC more than 3 years following her initial nephrectomy. The asymptomatic presentation of this patient with recurrent disease emphasizes the importance of close, long-term surveillance. More >
